Key Responsibilities
Resident Care

Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, oral care, toileting, and perineal care.

Provide mobility support, including transfers, ambulation, and use of assistive devices (walkers, wheelchairs, lifts).

Assist with meals by setting up trays, feeding residents as needed, and documenting intake and output.

Maintain resident comfort by making beds, changing linens, and keeping resident rooms clean, safe, and organized.
Observation, Safety & Infection Control

Monitor residents’ physical and emotional condition and promptly report changes (pain, skin issues, behavior changes) to nursing staff.

Measure and record vital signs, including temperature, pulse, respiration, blood pressure, weight, and oxygen saturation as directed.

Follow all infection control protocols, including proper hand hygiene, PPE use, and isolation procedures.
-
Maintain a safe environment by preventing falls, ensuring call bells are accessible, and responding promptly to alarms.
Communication & Documentation

Document care provided in daily logs or electronic medical records as required.

Communicate resident needs and condition changes to nurses in a timely manner.
-
Participate in shift hand-off reports to ensure continuity of care.
Support & Resident Advocacy

Provide emotional support, reassurance, and respectful care to residents and families.

Encourage resident independence while providing appropriate assistance.
-
Respect and uphold residents’ rights, dignity, privacy, and confidentiality.

Assist residents with participation in recreational and social activities.
Teamwork & Collaboration
-
Work under the supervision of an RN or LPN and follow delegated tasks carefully.

Assist nurses with non-invasive clinical tasks (e.g., collecting urine or stool specimens).

Support team members by answering call bells and assisting residents beyond assigned duties when needed.
-
May accompany residents to outside appointments as required.
