Licensed Practical Nurse (LPN)

Your Tasks

Resident Care & Clinical Duties

  • Provide direct patient care, including assistance with bathing, dressing, grooming, toileting, feeding, and mobility.

  • Monitor residents’ physical and emotional well-being and report changes in condition to the RN or physician.

  • Perform and document vital signs, weight checks, and skin assessments.

  • Administer medications (oral, topical, injectable, inhaled) per physician orders.

  • Provide wound care, dressing changes, and preventive skin care.

  • Collect specimens for laboratory testing and report abnormal results.

  • Assist with oxygen therapy, catheters, feeding tubes, IVs (as permitted by state regulations), and dialysis-related observations.

Supervision & Delegation

  • Supervise and direct CNAs in daily resident care activities.

  • Ensure adherence to care plans and proper documentation.

  • Perform skin observations during showers and pre/post-dialysis and report abnormalities to the RN.

  • Provide guidance, feedback, and support to CNAs.

Documentation & Communication

  • Accurately document all care, observations, and resident status changes in the medical record.

  • Participate in shift handoff reports to ensure continuity of care.

  • Communicate effectively with RNs, providers, residents, and designated representatives.

Care Planning & Team Collaboration

  • Assist with reviewing and updating care plans under RN supervision.

  • Participate in interdisciplinary care conferences.

  • Implement care interventions in accordance with care plans and physician orders.

Emergency Response & Advocacy

  • Recognize and respond to medical emergencies (falls, seizures, respiratory or cardiac distress) with prompt RN notification.

  • Assist with CPR and first aid as required.

  • Promote dignity, respect, and resident rights in all care activities.


Our requirements
  • Active LPN license in good standing

  • Experience in long-term care, skilled nursing, or veteran care preferred

  • Strong clinical, supervisory, and documentation skills

  • Ability to work effectively in a team-based environment

  • CPR certification required

Work Schedule & Pay Information

  • Holiday work: Required only at the request of the employer and paid at straight time

  • Overtime: Authorized only at the request of the employer and paid at the overtime rate

Shifts:
Day : 7:30 am to 4:00pm 
Evening 3:30pm - 12:00am
Night : 11:30pm to 8AM
